Hospital CHNA reports, implementation plans, progress reports

In December 2025, all Advocate Health Care hospitals in Illinois posted 2025 Community Health Needs Assessment (CHNA) Reports. A CHNA is an analysis of the population, resources, services, health care statuses, health care outcomes, and other data within a defined community or service area that helps identify potential health issues being experienced by community members. Every nonprofit hospital is required to complete a CHNA every three years under the Patient Protection and Affordable Care Act (ACA), to demonstrate that a hospital is committed to promoting health.

The hospitals’ 2025 CHNA reports are provided below for members of the community to view, print, and read. There is also an option to provide feedback via the email in each report, if desired, in compliance with the Patient Protection and Affordable Care Act (ACA).

Advocate Illinois hospitals developed 2023-2025 implementation plans, obtained governance approval and posted their plans prior to May 12, 2023. These plans outline strategies to address the key needs each hospital selected to address through their hospital CHNA process. Links to these implementation plans immediately follow each hospital's CHNA report. (The new implementation plans for the 2025 cycle will be posted by the next compliance date of May 15, 2026.)

In addition to the Advocate Health Illinois hospital 2025 CHNA reports and Community Health Implementation Strategy plans, the CHNA Reports and Community Health Implementation Strategy Plans for the previous cycles remain on this page, accessible for viewing by the community. All hospital CHNA reports reflect a 3-year assessment cycle. This dual cycle posting meets the requirements of the ACA.
